Death claim process in EPF
To make a death claim under the Employees' Provident Fund (EPF) scheme, the claimant (usually the nominee or legal heir) needs to submit the following documents:
Mandatory Forms and Documents:
1. Composite Death Claim Forms:
- Form 20 (for PF withdrawal)
- Form 10D (for pension, if eligible)
- Form 5IF (for Employees’ Deposit Linked Insurance (EDLI) benefit)
These must be duly filled and attested by the last employer
2.Supporting Documents:
- Death Certificate: Original
- Form-5/10: Employer to provide date of joining as well as date of leaving with reason for leaving
- Proof of relationship: Surviving family member certificate or list, attested by the employer (for married members, list of family/nominees).
- Identity and Address Proof: Aadhaar card and bank details (of nominee/legal heir/claimant), attested by employer.
- Photographs: Three passport-sized photographs of each claimant, attested by employer.
- Cancelled cheque/copy of passbook of the claimant (for direct credit to bank account).
3. Situational/Additional Documents:
- FIR/Post-Mortem:In case of Unnatural death
- Guardianship certificate (if the claim is made by someone other than the natural guardian of a minor).
- Succession certificate (if no nominee is registered or claim is by legal heirs).
- Non-employment certificate (declaration that the deceased was not employed elsewhere).
- Dependency certificate (in case claim is by parents and member was unmarried).
- No objection/relinquishment deed (if one of the legal heirs does not wish to claim; not always mandatory).
- Birth certificate/school certificate of children for pension claims
The above documents need to be submitted to the Regional Provident Fund Commissioner of the region where the last employer of the deceased member had maintained their Provident Fund account and transacted PF business. This is the office responsible for processing the death claim and disbursing benefits such as PF balance, pension, and EDLI insurance to eligible claimants. Ensure that all required forms and supporting documents are complete and properly attested before submission for a smooth claim process.
